Packing Slip

Please send in a packing slip with each repair order and a problem description with each item in the order.

The packing slip should contain:

Return name address and phone number.

Payment choice

bullet

Return shipping requirements

bullet

Any special requests

Please send in problem descriptions whenever possible! Even if the problem seems obvious to you.. "DEAD".. it could easily be an intermittent problem and the item can work just fine on the test bench. The more troubleshooting you do (and the more information you can give us about the problem) increases the quality of repair work we can offer you.
